EPA AGE GROUP ENTRY FORM

For those wanting to compete in the EPA Age Group gala in FEB, Entries are due on Tuesday 15th of January, no later than 5pm. Head Coach has adviced those knocking on Level 3 QT, to look at going to this gala.
You may pay the Gala Entry fees by EFT into the Ultimate Swim Club account. Reference to be used is : EPA6/Surname of swimmer. If paying by EFT, please attach proof of payment to Entry forms submitted. Alternatively you may also e-mail the Entry Form & Proof of Payment to smitkaren5@gmail.com
